The New South Wales (NSW) government has declared its intention to adopt all suggestions put forth by the coronial inquest regarding the Westfield Bondi Junction incident. This announcement follows six months after the coroner delivered her findings, which included 23 recommendations, 13 of which were specifically directed at the state government and its agencies.
The coroner, Teresa O’Sullivan, noted that the attack was perpetrated by Joel Cauchi, who was described as “floridly psychotic.” On April 13, 2024, Cauchi fatally stabbed six individuals—Yixuan Cheng, Faraz Tahir, Jade Young, Pikria Darchia, Dawn Singleton, and Ashlee Good—and injured ten others in a violent episode in Sydney. He was shot dead by police shortly after the assault commenced.
In her inquiry, Magistrate O’Sullivan sought to uncover systemic flaws and opportunities to enhance state responses to such crises. The government has publicly expressed its commitment to implementing 11 recommendations in full, while supporting two others in principle.
Among the recommendations, the coroner urged collaboration between the NSW Police Force and the Queensland Police Service, along with the Royal Australian and New Zealand College of Psychiatrists (RANZCP), to evaluate the role of psychiatrists in assessing fitness for weapon licensing. A working group has since been established to tackle this issue.
O’Sullivan emphasized the urgent necessity for short-term housing solutions in Greater Sydney for individuals facing mental health challenges and homelessness, while also advocating for long-term accommodation strategies. The NSW government indicated that agencies like NSW Health and Homes NSW are prioritizing investments in housing initiatives such as Habilis, HAVEN, and Pathways to Community Living.
The coroner also called for the government to seek guidance from NSW Health regarding the decline in mental health outreach services and the resources needed to address the gap in outreach psychiatric care. Furthermore, she highlighted the importance of ensuring that clinicians at community health centers have access to up-to-date guidance related to the cessation of psychotropic medications.
Another significant recommendation urged the NSW government to explore the implementation of co-responder models, pairing police with mental health professionals during emergencies. Recently, it was confirmed that healthcare workers would now assist individuals facing mental health crises instead of police intervention.
In light of Cauchi’s affinity for knives, the coroner suggested that the government evaluate whether current laws concerning police knife detection procedures should be expanded to include certain crowded areas without needing a formal declaration. The NSW government has already enhanced penalties for knife-related offenses and broadened police powers for knife scanning.
Additionally, O’Sullivan proposed an awareness campaign titled ‘Escape. Hide. Tell’ aimed at educating the public on surviving encounters with armed offenders. The NSW government has committed to developing a comprehensive communication strategy to support this initiative.
To improve emergency medical responses, the coroner recommended that NSW Ambulance incorporate Tranexamic acid into their standard equipment for emergency vehicles. This medication, crucial for controlling severe bleeding, has already been implemented in paramedic practices.
Furthermore, the coroner advised that the ongoing review of the major incident response plan by NSW Ambulance should include an evaluation of command roles, training adequacy, and the introduction of a new identification system for commanders at incident scenes. The government has stated that a revised response plan is undergoing final assessments.
Lastly, NSW Ambulance was encouraged to expedite its review of the special operations unit’s response capabilities, with the agency reporting that it has made significant progress in this area.
