The state government has announced an investigation into a troubling incident that occurred in Melbourne’s western region, where a woman underwent a surgical procedure intended for a different patient.
According to the woman’s attorney, she awoke at the Joan Kirner Hospital in Sunshine this January to discover that an incorrect operation had been performed on her.
Western Health, the facility in question, stated that it cannot discuss specific cases but expressed its commitment to transparency with patients throughout the investigation process.
The incident involves Debra Buchanan, who visited the Joan Kirner Women’s & Children’s Hospital in January for a minor operation aimed at removing a lesion from her pelvic area. However, her attorney reported that she instead emerged from surgery with a section of external genital tissue removed, resulting in permanent scarring.
Erin Monsalve Fear, Ms. Buchanan’s lawyer, described the situation as a “complete systemic failure” on the hospital’s part. She highlighted the numerous safeguards and protocols that should have been in place to prevent such an error and noted that these measures either failed or were not adhered to in Ms. Buchanan’s case.
In addition to the physical pain and lasting disfigurement, Ms. Monsalve Fear mentioned that her client has lost all trust in the healthcare system. “Deb has had to undergo further surgery to address the cancerous lesion that was originally meant to be treated,” she explained. “This ordeal has understandably left her anxious about returning to the operating room.”
Ms. Monsalve Fear indicated that her client is pursuing damages and hopes to raise awareness by sharing her experience.
The Department of Health has confirmed that Safer Care Victoria, the agency responsible for monitoring patient care across the state, is reviewing the incident. Government Minister Vicki Ward described the occurrence as “extraordinary” and expressed her sympathies towards Ms. Buchanan, labeling the situation as unacceptable.
“What has happened to her is truly distressing, and it should never happen,” she stated. “A review is underway, and Safer Care Victoria is collaborating with Western Health on this matter.”
John Ferraro, the chief operating officer of Western Health, issued a statement affirming the organization’s dedication to maintaining open dialogue with patients during the investigation. However, he noted that due to patient privacy laws, the hospital cannot provide details about individual cases.
Ferraro added, “In accordance with Victoria’s Statutory Duty of Candour, when an adverse event occurs, we are obligated to apologize, meet with the affected patient, conduct a review of the incident, and communicate the findings and preventive measures.”
Despite the surgery occurring in January, Ms. Monsalve Fear stated that Western Health has yet to extend an official apology to Ms. Buchanan. “While there has been some acknowledgment from the hospital that the incorrect procedure took place, it has not translated into a formal apology for Deb,” she remarked.
