Sylvester Stallone’s impressive physique has long been as recognizable as his legendary roles as Rocky Balboa and John Rambo. Recently, the actor disclosed the extent of the physical toll he endured to attain these iconic appearances.

In an interview with The New York Times, Stallone discussed the transformation he underwent for films like “Rocky” and “Rambo,” revealing that his focus on his body evolved into an obsession. “It became very intimate, almost like armor,” he explained, recalling how his relationship with his physique transformed over the years, particularly since he wasn’t particularly muscular prior to these roles.

“I began to envision my body as designed like a jaguar—sleek and agile rather than large like Arnold Schwarzenegger. This feral and athletic idea became an obsession, one that spiraled out of control. It was so severe that I developed bulimia,” he stated candidly.

Bulimia is characterized by episodes of binge eating followed by purging, which can be life-threatening. Stallone’s obsession with maintaining a low body fat percentage quickly escalated. “During the filming of ‘Rocky III,’ I got down to 2.6 percent body fat and struggled to keep anything down. My fixation on fitness turned into an obsessive worship of my body,” he confessed.

Despite the alarming figures, Stallone’s determination only intensified to maintain his physique. Dr. Manjusha Agarwal, a Senior Consultant in Internal Medicine at Gleneagles Hospital in Mumbai, commented on the dangers of such low body fat, labeling it “extremely dangerous and unsustainable.”

“A body fat percentage of 2.6% is not a fitness goal; it falls into a medical emergency category. When combined with bulimia, it poses severe health risks that require immediate attention,” she informed Indian Express.

The risks associated with such low body fat levels and bulimia are significant, as outlined by Dr. Agarwal:

  • Heart Risks: Low body fat coupled with frequent purging can lead to electrolyte imbalances, increasing the likelihood of irregular heart rhythms, cardiac arrest, and sudden death.
  • Organ Failure: The body starts to deplete muscle, including vital heart muscle, for energy, which puts immense strain on the kidneys and liver.
  • Hormonal and Immunity Collapse: Hormonal levels plummet, bones weaken, immunity decreases, and the body struggles to maintain temperature regulation.
  • Digestive and Metabolism Issues: Chronic vomiting can inflict lasting damage on the esophagus, stomach lining, and teeth, while severely disrupting metabolism.

Stallone also candidly admitted to using steroids during this phase of his life. “No one achieves this without supplements or drugs, period. It’s unrealistic to think you can maintain such a physique while consuming only yogurt and pudding,” he remarked.

Reflecting on his experiences, Stallone expressed that this relentless pursuit “took a terrible toll,” leading him to experience significant health setbacks, including nine back surgeries, two shoulder operations, and three neck fusions. Given his intense lifestyle, these issues were not entirely surprising.

Dr. (Col) Vijay Dutta, Director of Internal Medicine and Respiratory Services at ISIC Multispeciality Hospital, noted that a prolonged commitment to such an extreme lifestyle can result in chronic stress on the joints and spine, leading to persistent pain and degenerative conditions.

While Stallone acknowledged his use of steroids, Dr. Dutta indicated that individuals striving for extreme muscularity or minimal body fat often resort to additional substances, such as growth hormones, testosterone boosters, fat-burning stimulants, and diuretics to drop water weight.

“Although these may yield rapid changes, they come with significant health risks, especially without medical oversight,” he warned.

Dr. Dutta emphasized that the use of anabolic steroids amplifies health risks, potentially resulting in high blood pressure, heart disease, liver damage, hormonal imbalances, infertility, and mood disorders, all of which can adversely affect both physical health and overall quality of life.

Dr. Manjusha also identified several severe physical and emotional symptoms associated with extreme bulimia:

  • Physical Symptoms: Frequent sore throats, damaged teeth, and swollen cheeks from repeated vomiting, chronic stomach pain, dehydration, and irregular heartbeats are common signs.
  • Emotional and Behavioral Indicators: An obsessive preoccupation with weight, secretive eating habits, extreme mood swings, and social withdrawal are also prevalent.

While Stallone pursued an ideal physique for his film roles, his narrative illustrates how swiftly such ambitions can become detrimental to health. Achieving a body fat percentage of 2.6% alongside bulimia pushed his body far beyond safe limits.

“Sustainable fitness is best achieved through balanced nutrition, appropriate training, and sufficient recovery, rather than reliance on performance-enhancing substances,” Dr. Dutta concluded.
