The Australian Transport Safety Bureau has reported on a freight train derailment that occurred in May 2023, involving a train traveling between Brisbane and Sydney near Kyogle in northern New South Wales.<\/p>\n
According to the investigation, the incident caused the derailment of 11 wagons on a stretch of track known to have ongoing issues. In response to the findings, the authority responsible for managing the freight rail network has centralized its data concerning asset conditions and maintenance records.<\/p>\n
The derailment, which temporarily halted rail services for more than five days, involved a Pacific National freight train consisting of three locomotives and 56 wagons. On May 31, 2022, an empty wagon’s undercarriage derailed, leading to the bogie dragging along the tracks for a distance of 2.3 kilometers before a total of 11 wagons came off the rails, triggering an automatic emergency braking system.<\/p>\n
The Office of Transport Safety Investigations (OTSI) conducted the inquiry and confirmed that there were no injuries; however, the damage to the wagons and track was substantial. OTSI’s Chief Investigator, Jim Modrouvanos, indicated in his report that, although there was a temporary speed limit at the derailment site that the train crew adhered to, a partial repair had been made just four days before the incident.<\/p>\n
The investigation revealed that the driver had slowed the train from 70 kilometers per hour to a speed between 16 to 20 kilometers per hour upon receiving a warning. It was noted that the track had only undergone standard inspections and had not been subject to more thorough evaluations.<\/p>\n
“The issues likely worsened since the previous inspection,” Modrouvanos stated. The OTSI findings highlighted that the measures taken to address the defect had been largely informal, contributing to the incident.<\/p>\n
The derailment caused significant disruption, resulting in the closure of the rail line for over five days while repairs were conducted.<\/p>\n
The Australian Rail Track Corporation (ARTC), which oversees more than 9,600 kilometers of rail across five states, including South Australia, Victoria, New South Wales, Queensland, and Western Australia, was responsible for the track’s maintenance. Modrouvanos pointed out that ARTC failed to utilize its asset management system or the data collected before and after the temporary repairs to identify the need for extensive work.<\/p>\n
“Such information could have enabled ARTC to make informed decisions about necessary major repairs, potentially leading to more effective long-term solutions,” he added.<\/p>\n
In light of the incident, the report indicated that ARTC has since established a new centralized approach to evaluate asset conditions, incorporating data, images, and maintenance records across its network.<\/p>\n
Modrouvanos emphasized the importance of robust asset management practices within a safety management framework, stating that it is essential for operators to ensure that qualified personnel in critical safety roles consistently adhere to established procedures and standards.<\/p>\n","protected":false},"excerpt":{"rendered":"
