The Department of Justice and Community Safety has reported that a coroner has issued 18 recommendations following the death of a man due to mixed drug toxicity while residing at a facility for sex offenders.<\/p>\n
Coroner Audrey Jamieson determined that the man’s death could have been avoided and criticized Corrections Victoria for its inadequate support of his rehabilitation process.<\/p>\n
In her findings, Coroner Jamieson advised that Corella Place should take responsibility for managing the medication prescribed to residents.<\/p>\n
An inquest revealed that the residential facility in regional Victoria permitted a resident to have control over his prescribed medications, which was a contributing factor to his passing.<\/p>\n
The individual, in his 30s and whose identity is protected for legal reasons, was serving a post-sentence order at Corella Place in Ararat when staff discovered him unresponsive in his unit in May 2020.<\/p>\n
Corella Place is a secure facility operated by Corrections Victoria, designed for individuals who have completed their prison sentences for sexual offenses. While it is not a correctional institution, residents must adhere to the conditions set forth in their post-sentence orders as mandated by the courts.<\/p>\n
Coroner Jamieson noted that the deceased was particularly vulnerable and did not choose to reside at the facility voluntarily. The report highlighted that he suffered from ADHD, depression, anxiety, and had a history of self-harming behaviors.<\/p>\n
Ms. Jamieson concluded that the man’s death was not a deliberate act of self-harm but rather a consequence of mixed drug toxicity.<\/p>\n
At the time of his death, the man was under COVID-19 quarantine, confined to his unit and a small outdoor area for 23 hours each day, with just one hour permitted for physical activity.<\/p>\n
Clinical forensic medicine and addiction medicine consultant Edward Ogden, an independent expert consulted during the inquiry, described the combination of boredom, isolation, and despair at Corella Place as a “perfect storm” for drug use. He noted that, based on his experience with patients under court orders or those who have been incarcerated, drug use often serves as a coping mechanism to alleviate the monotony of life.<\/p>\n
Ms. Jamieson stated, “I find that there was a direct link between Corella Place’s failure to adequately monitor and control the entry of methadone tablets into the facility and the circumstances surrounding the man’s death. Furthermore, Corrections Victoria did not provide the necessary support for his rehabilitation.”<\/p>\n
She also remarked that the man was not treated with the dignity and respect he deserved, considering his brain injury and ADHD.<\/p>\n
In response to her findings, the coroner made 18 recommendations, with 16 aimed at the Department of Justice and Community Safety. Three of these recommendations specifically address residents’ management of their prescribed medications.<\/p>\n
Coroner Jamieson recommended that residents should be required to inform staff of any medications that could be misused or abused, and that these medications should be administered by personnel. She suggested implementing a medication dispensing system similar to those utilized in correctional facilities.<\/p>\n
Additionally, she called for a review by the Victorian Equal Opportunity and Human Rights Commission regarding the training provided by the Department of Justice and Community Safety related to human rights.<\/p>\n
Other recommendations included increasing family visitation opportunities and mandating training for staff to assist residents with disabilities and to identify signs of drug misuse and overdose.<\/p>\n
